Looking to extend my Driveway another 3 ft wide by like 15 ft. Was thinking I could do it myself, but anybody know what the going rates are for something like this, so I don't have to kill myself.

that is about a half yd of concrete, not much to pour but expensive to order that small amount.
5,000 psi sacrete with some extra portland added will work great but also a pain to mix that much (each bag is only about 1/2cf)
about 30 bags of sacrete, 2 bags of portland (split this evenly as you mix to strengthen the sacrete), rent a masons trough if you can to mix in and you could do it easy enough. still gonna be hard work though.

I just finished this...Roughly 5'X16"..I used 24 bags of Maximiser Mix..I have my own electric mixer..

On just the square footage that would normaly run you about $250 for 45 sq ft. Since it is such a small amount you will pay a contractor a premium. I would guess $500 +.
If you are up to it your self do it like Hop suggested. Magerial will probably run you about $150+.

see if you have a cement or rental yard nearby that has a portamix trailer setup. they mix it dump it in the trailer you drive it home and do the work, I use to do small projects with friends all the time that way. the buggys would usually hold up to 1 yard. You have a little over 1/2 yard as was mentioned.

not only dowl it, but i would add a hearty footing along the outside edge... at least one foot down if not more.
