Benefit Fishing Tournament for 18 Month old Teagan Choate of Buna, Texas. Teagan was born with a Lobar Hiloprosencephaly, and several conditions stemming from this diagnosis, including a cleft lip/palate.
All proceeds will go to Teagan for medical bills and upcoming surgeries. There will be things for kids, Plate lunches for sale, Silent Auction, Door Prizes,

Having acquainted myself with the rules, I/we have completed this application and I am enclosing my entrance fee of $50 Single or $100 Team (CASH ONLY) in signing this application, I/we hereby waive and release all other contestants, the host sponsors and tournament officials from all claims or injury and or damage incurred in connection with this tournament. I/we further agree that the Tournament Committee reserves the right to refund this entrance fee if it shall choose for any reason not to accept my/our application. I/we swear that the above facts are true and accurate to the best of my/our knowledge.

Rules and Requirement
1. You cannot start fishing until 6:00 am.
2. Entry form with complete entry fee must be received before fishing.
3. All fish must be caught during tournament hours on Lake Sam Rayburn.
4. You must be in the weigh-in line by 3:00pm
5. Official time will be CELL PHONE time.
6. No trolling with the main engine.
7. Trailoring is allowed. (Bank fishing is permissible)
8. Weigh-in will be at Umphrey Pavilion at 3:00pm.
9. All kids CAN fish with live bait
10. Only (5) BASS can be weighed in per. team.
11. All BASS must be 14� in length to be weighed in.
12. All State & Federal penal code laws shall apply at all times.
13. US Coast Guard approved life jackets are required for each person in boat

This should be a pretty big tournament. Bigger than anticipated would love to see some folks from here out there to support a great cause.
