I get a DOT Physical every year. And every year they have said I'm obese. This year I finally spoke up. I said my chart says " I'm thick and sexy " .

Everybody thinks they’re 4-5 pounds from being Mr. Olympia. What’s your body fat percentage? As mentioned, get a Dexa scan. They’re relatively cheap and pretty accurate. They’ll give you much more information than BMI, but I suspect the results will backup what your doctor says.

BMI is pretty accurate, too. It becomes less accurate for professional body builders who are 8-10% body fat at 5’9” 230 lbs. I know you workout six times a week, but is that what you’re working with? 8-10% body fat? Being that muscular at that body fat percentage comes with health issues, too, and they mimic what being obese does to your body (sleep apnea, blood pressure issues, heart rate issues, etc. It’s extremely taxing on your body to keep that amount of lean muscle mass on, even if your body fat is low).

I was obese five years ago. I thought I needed to lose 20 pounds. I was still obese. I did that two more times (total 60 pounds down) and I’m at the upper end of normal BMI. One weekend of beer and pizza and I’ll be back above 25 BMI for a week until get back in the gym and on my normal diet.

My parents both say I look skinny every time I see them. I look about what I should look like and I could still stand to lose 8-10 more pounds of fat. Americans have lost sight of what normal, healthy body weight is in a lot of cases.

I don’t know you, but your story sounds a lot like what I went through five years ago when I decided I needed to lose weight. I’m currently 5-6” smaller in waist size, sleep better, move better, don’t get tired during the day, and look better.

It’s a shock to the system to come to the realization that the mirror, the scale, our family/friends, sometimes lie to us.

Inbody scans are another good option. Dexa is the best, but can be harder to find. But in any case, the real answer is Body Fat and Muscle Mass analysis and body segment symmetry. If you are trying to slay a bunch of chicks maybe add in chest to waist ratio. Get your body fat down to around 15%. Get your muscle mass to body weight % above 50% and get your chest to waist ratio to about .7 and you'll be healthy as an Ox and fighting chicks off with a stick.

BMI is a close to worthless metric in my opinion.

Lot of old people in the community I live in. You don't see too many fat guy oldsters out there fishing. The few really old dudes that are still fishing are always skinny guys. I'm convinced if you want to live long and still be functional in old age, you need to be skinny.

I'm convinced you need muscle mass. You lose 1-2% per year after the age of 60. You need a certain amount of muscle to be able to do most activities from hiking to being able to get in and out of a boat and balance. How much muscle you need depends to some degree on how fat you are. Fat guys need more muscle to do basic tasks. Skinny guys need less. Get an inbody or dexa scan done to determine how many lbs of muscle you have. After that it is relatively straightforward to do the math on how long you are going to be able to maintain an active lifestyle. Bare minimum you need 30% of your weight in muscle to stay active. Ideally 40%. If you are 60 years old and 50% of your weight is muscle, you've got at best 10-15 years max if you are not actively intervening. (Testosterone, protein, and resistance training.) Most 60 year olds are not at 50% muscle mass so the decline happens much faster than most anticipate.

I changed the way I eat this past August to get my cholesterol level down. Mission accomplished, cholesterol dropped like a rock. I also lost 35 lbs as a result. I am 6’4” and weigh 190 lbs. Feel great, feet and knees don’t hurt anymore. Biggest problem is I can’t keep my pants up. This is causing me to be a premature suspender wearer even though I bought new pants to match my new weight.

BTW if anyone has a better solution to keeping pants up than suspenders or cinching my belt so tight I can’t breath, please let me know. Wife doesn’t like the suspenders, I don’t really either but it’s the only option I have right now.

Family friend who was a lifelong smoker lived to be 92. He was very skinny. He did have to drag an oxygen deal around the last few years so there's that. Genetics play a big role too.

The best indicator of health is the BMI Body Roundness Index. Your measurement around the largest part of your belly should be half or less than your height.
