Your medication(s) were processed by the retail pharmacy using the Maximum Allowable Cost (MAC). This is a pricing strategy which establishes maximum retail provider reimbursement for multisource products, on a product-by-product basis.

The process used to calculate costs to TRICARE and the beneficiary are as follows:

1. The total cost or billed charge of the claim is determined by calculating the Maximum Allowable Cost paid by the (OHI) and the patient copay.

2. TRICARE determines how much they would pay if they were the Primary Payer; this amount is calculated by determining the drug cost and then subtracting the applicable TRICARE copay. This is the TRICARE Maximum Allowable Cost.

3. Reimbursement to the pharmacy and the remaining copay to the beneficiary are determined by the billed charge and the amount remaining after the primary coverage (OHI) has paid its benefits.

If the amount due is less than the TRICARE Maximum Allowable Cost, TRICARE will reimburse the pharmacy up to the remaining amount. The beneficiary will have no copay.

If the amount due is greater than the TRICARE Maximum Allowable Cost, TRICARE will reimburse up to the Maximum Allowable Cost. The beneficiary will be responsible for the remaining cost up to their TRICARE copay.

If the (OHI) paid amount is greater than the TRICARE Maximum Allowable Cost, no further reimbursement is due the pharmacy. The beneficiary will have no copay.

As an example for your claim the amounts can vary based on the contracted price at the time of filling:

Date of Service: 01/20/20
Ingredient Cost Submitted: $537.58
Cost Approved: $418.10
Primary Payer Amount Recognized/Approved: $346.53
Tricare billed: $71.67
Total Member's Cost: $0.00


This particular prescription was $0 out of pocket first 2 times it was filled but the third time it was filled cost $105 out of pocket. If the cost of the medication raises and the amount is over the Tricare maximum allowed you get to pay the difference. Also Tricare will not pay more as secondary insurer than Medicare paid as primary insurer.

There are some docs that dont accept Medicare but it seems like they are usually not with any large group of doctors in their practice. I turned 65 eight years ago and Part B was $108 then but as mentioned is now $144. The doctors grumble because Medicare wont pay them $300 for a 10 minute office. Its Their Way ($$$$) or The Highway! Which is good or bad depending on which side of the fence you are on. The Supplement is the key for anyone on Medicare. If you have TriCare for Life thats even better.

So the last caveat I learned to Medicare:

Every calendar year you will initially have to pay $198.00 (deductible) of your Doctor's visit payment before Medicare and your Supplement will pick up the rest of your Doctor visits cost for the remainder of the year.

This is not necessarily true. I have Humana ($92/mo.) and have never had it turned away here at any hospital, doctor, or pharmacy. There may be restrictions, but I have not encountered one after 10 yrs. on the plan. All my drugs are included at no extra cost to me through their Humana pharmacy. If I went to a lot of drs., I would look around due to the $40 copay for a specialist. However, I don't at this time. You have to look around and determine what is best for your individual needs. I do know a lot of folks who pay too much for what they actually need.

Has anyone shifted from advantage to medicare + medi gap? Is that effortless or a beating.

If you are still working and have insurance you only need to get part A. Its no cost.
Within 30 days, they told me prior is best, of losing your insurance you need to get on part B. Failure to do that in this time period may increase your cost for the rest of your life.

If you are already drawing SS they automatically sign you up for part A.

I'm three years away from retirement. By that time Biden will have passed Medicare for All and all you youngs will be paying for my free healthcare. grin Yep, I'm just Biden my time.

To avoid the increase in premiums I had to prove to them I had viable insurance (they have a form). If you work for a company with a faceless nameless behind 1-800 numbers this is a daunting task. I had to call in favors to get a simple form completed.

I guess I am ignorant to all this, but I thought when you turned 65 you signed up on medicare and any insurance you may have with employer became a supplement? If none then you have to buy the part A,B,C, or whatever if you want it?
