I would love to know what the definition of a “case” is according to reporting agencies. Total number of positive tests and number of cases are not the same thing.

I remember the denial that was prevalent in the 80's concerning AIDS.
Then, just like now, during the early stage of the virus those who were the most skeptical were often the people who were the most at risk.

Where I am at, they count ones that have test results back as Positive Cases, and they add Presumptive Cases (defined by the person having had known contact with a person with positive test results, having symptoms, but don't have their test results back yet) to the Positive Test numbers to get the total number of cases they are tracking.

Note, they don't get the Presumptive here unless they are showing symptoms. So far, most if not all get move to Positive when their test results come back.

On a related note, they no longer report recovered here because of the lack of guidelines saying what recovered is. At one time they were saying that it was 10 days after a positive test with 10 or more days of improving symptoms. Not sure they know for sure how long a person is contagious. maybe that is why they have trouble reporting the recovered numbers. When my sister in Kansas was exposed to a person with COVID, she had to self isolate for 14 days, they did a test, it was 26 days later that she got her test results back. She tested negative luckily. She is in the higher risk for death pool if she ever gets COVID 19.
