Hydraulic jackplate- Bob's Machine
Manual jackplate- Rapid Jack
If you get a hydraulic plate, invest in the dash gauge to set you height.
Just my opinion, I am sure you will get others.

RBelott, Bob's has manual and hydraulic plates in different setbacks. R&R design has manual and hydraulic plates in 2 or 3 sizes, and they are located in Terrell, TX. I have a CMC manual plate, and it has been extremely strong. It has 5.5" of set back, which is enough for my setup. It is made in OK.
Any shop in the area that works on boats or engines should be able to order a jackplate for you. Call around and see who will make the best deal. If you know how much setback you want, you might also try Screamandfly.com and Bassboatcentral.com. They both have Sell/Swap areas where you can find parts.

If it's a motor that's still under warranty you might want to check if the warranty is still good if you install a jackplate.
