Ok this is kind of a stupid post but I've gotta ask it.
I was at fayette on Sunday (which was great by the way) and on several fish that I hooked , they were not hooked deep at all. Many of them didn't even get to the barb. I was fishing a 7 in power worm with a 3/0 gamakatsu ewg hook (it was new that day, never used) on 15 lb P-line cx. My worm rod however was apparently over looked when I got it because it is a medium with a fast tip. I'm getting a couple of MH and H for myself around christmas.
Do you guys think the shallow hooking was the size of the hook, just a dull hook in the pack, the rod power, combo of all of them? I seriously was dumbfounded that some of those fish got in the boat.

I would guess that the rod is too soft an action for setting the hook good especially if you have it tx. rigged instead of having it tx.posed.
Switch to a M/H rod.

I'm going to say your it's your medium action rod. I use the same hooks you using with very good results. A rod with a little more backbone will help. In the mean time you might try a line with little to no stretch. By the way that's not a stupid question. Good luck to ya.

Cool. I was hoping it was just the rod. Thats an easy fix. I will just use some fluoro until the new rods come in.

Yep, Medium action is too flimsy for work fishing, in my book atleast.. MH-H only

Can't use a buggy whip on those green fish unless you're cranking. It's the rod.

could just use braid main line with a fluoro leader and a 4/0 and rock it if you like the rig *shrug* Matzuo J sickle, texposed... fish hook themselves. You can use a mickey mouse pole and still get em hahahha

If you had any amount of line out, I'd say the soft rod and mono were the culprits...

I can drive that same hook all the way through a fish's head if it hits in the right spot.
It's not the hook it's a combination of rod action,power , line choice and conditions more than likely.
If you are short linin fish(ie close to the boat flipping) you might be able to get away with a medium in that instance, but might still give some issues.
The deeper you are fishing, the further away you are casting(how much line you have out) will impact the hit of the hook on the fish.
I never tx rig with anything lighter than a mh, but also lenght comes into play here too.
A good 7'mh with 15lb flouro will get you better results to a point. If you are fishing deep and or long casts with alot of line out you will find a 7'3-7'6" rod will take up line and drive the hook better, and might need to stiffen up rod at some point too.
I have missed some key fish with a good 7'mh when fishing very long casts deep that switching to a 7'6" h resulted in good hook sets
So take into account the environment you are fishing in as well.

Never had an issue with Gamagatsu on sharpness, I use the same hook with mono no problems. Like everyone above said, MH minimum when t-rigging, shallow, H when in deeper water.
I am not making any decisions until the bourbon kicks in!

Another issue not covered above: What plastics were you using? If you were throwing a brushhog, that hook is too small, the brushhog will ball up and you will not get a good hook up resulting in lost fish. Bigger plastics bigger hook, use the 3/0 for smaller finesse rigs like centipedes, baby brushhogs or smaller floating worms.

All hooks regardless of brand are proned to possibly be dull straight out of the package. But, I think your issue is rod and length of line.

What action is your tip on your rod? Mod-fast will have less power for a hook set than fast - extra fast
