Besides the "obvious" pressure washing off the existing grease, how else does one need to disassemble to get that out of there?

I use carb cleaner to clean that nasty [censored] off of cables. It's going to take a lot, you've got years of gunk built up, then use penetrating oil and keep working it until it's smooth and free.

Just what's visible on the shafts? Or do I need to remove the nuts to get inside?
