So far in the last ten of so years, I have had two (2) bass to my recollection that have died. Best I remember those bass were caught on deep cranking in about 25' of more of water. I did not have my fizz needle then so they died from that I suppose. I have not had any die but those two and here in Alabama from July to late September its not uncommon to have those 90+ degree days.
I freeze old soda bottles and throw them in there when needed. I keep them in my boat ice chest until they are needed. If I catch a fish from 15' or deeper on hot days I fizz them with my needle kit. I have those weighted fin clips for those I even suspect might be affected but most times i just fizz them. It takes maybe 2 or 3 minutes and I'm good to go.
I keep my live-wells on automatic re-circulation also.

I use ice, dechlorinator, and fin clips on deep fish.

Weighed in a few that were caught in 30'last weekend that were able to swim down upon releade with no issues.

Used to fizz them, but figure if i dont have to poke a hole in them and i can keep them upright in the livewell they are better off.

Ive lost one 12" fish on Ivie a couple years ago that was hooked in the top of the mouth on a senko that was caught in 5'. Went to cull it a couple hours later with a 3+ and it was dead. Had a 6 and a 4 in the livewell that were caught deeper and they were fine. Sometimes you just cant explain it. It was the only dead fish so far ive weighed in.

Most of the ice I see in gas stations/stores is not chlorinated. Generally its made from distilled water so you shouldn't have to worry about the chlorine.

Proper fish care is not only to weigh in your fish alive but to ensure they live beyond release. For anyone that doesn't run pure oxygen, you should look into it. For the cost of a nice rod/reel combo ($250-$300) you can have a an oxygen system. The refills are cheap and simple like exchanging your propane tank.
