After you set a brush pile, how long do you wait before you fish it? Is there a rule of thumb or do you just keep graphing it until you see fish holding on it?

It completely depends on the population density of fish and the volume of water they are in. In a tiny 1 acre pond they start associating with structure very quickly, but in a larger lake or river it make take a few months before fish occupy it heavily. Also, if you put it in an area where there is little fish locomotion, they may not ever occupy it consistently. Its good to put it in a spot where fish already travel. Generally it takes a few weeks for algae to cover the brushpile, which subsequently attracts small fry and in turn larger fish.

Depends on what type of brush you used. Willows tend to hold fish sooner than other trees imo

Roughen up the limbs (natural or PVC/artificial) with some 100 grit sandpaper. Gets the microbes and algae adhering and growing faster.

If its a spot holding fish frequently, within a day. If you are "creating" a spot its trial and error if they will even use the pile. Put out a bunch, a few will produce typically
