This is what I've been using for a couple years now and love it. It not only allows you to use the phone while inside the bag, it also floats. Only thing is you cant take pictures because of the blue back.
I have the Dry Pac and have never had a problem with it. Mine is a bag type and you can take pics through it. It's clear and has a strap you can hang around your neck if you like.

I used the dry pac last year and I got a pin sized hole in it while wade-fishing. It only let in a small amount of water but it was enough to ruin my phone. I now keep my phone in a regular zip-lock bag inside of a small plano case. The case keeps it protected and I can pull it out and use it through the zip-lock bag when I need it.

I've been seriously thinking about going to this case. http://www.lifeproof.com/shop/cases/iphone-4-4s/ Its currently in a OtterBox but its a bit bulky when I put it in my small drybag. The Lifeproof case seems pretty legit. There is quite a few youtube videos on it as well.

I've been using one of these for 4 years through a lot of rain storms and wave spray with no problems.
I bought the similar bag at Academy that is slightly larger (and $1 cheaper). Due to the larger size, I don't keep it around my neck, but it is clear on both sides so I can use the iPhone camera if I want as well as use the phone features without removing it from the bag. Size wise, it will hold my phone, wallet, and keys if needed.

I will vouch for the LifeProof case as I mentioned in my post up the page.
I also use it in something that is tethered to me or will flost as it will sink if dropped.
