I've always had good luck with a regular snell knot.

I use the palomar knot to tie 80# mono leader to a swivel.

I've started tying all my leaders with a surgeons loop, then I tie my swivel to my mainline, run my leader through he swivel, and loop it back.

I'm not sure that's the proper name for it, but the way you tie it is very similar to a snell. You run the tag end through the eye and back up the main line, make a loop with the tag end against the main line, wrap the tag around the mainline and the loop, and then pass the tag back through the loop and pull the main line tight. Explaination probably doesn't do it justice, but it's a knot my dad taught me when I was a kid, and I have never had a failure at the knot.

Palomar knot is a very good knot that is easy to tie. I snell the hook and then Palomar knots for everything else.
