It looks like fish get many of the same or similar ailments in humans..might be a tumor. Whatever it is, I wouldn't put it under a knife and eat it.

It's uhh CUZZIN to Blinky thuh 3-eyed fish thet lives in Springfield, right down the road from Montgomery Burns' power plant! They be friends!

The fish has a tumor. Without a biopsy and histology you can't say for sure whether it's a benign or malignant; however since the fish looks otherwise healthy I would say a contained benign tumor or cyst. The fish will probably live out a normal lifespan unless the tumor gets so big it interferes with its vital organs.
