It can be wrote on the jug or just on a plastic tag. It does not matter. Just has to have the info attached to the jug any way you can get it.

Actually a plastic card holder may not be sufficient in some cases. The wording of the law states that the "tag" must be as durable as the float.

Most people just write it on the float it's self.

FWIW, Last weekend a GW checked my jugs, and he was able to see all the information on the float it's self so he didn't mess with pulling them out of the water. It was rather nice to have it done that way for me since he checked me while I was in the middle of checking and removing fish.
