Transom plate??

Any one tell me where to find a plate that goes inside of the top part of the transom that the jack plate or motor bolts go through. Not sure I have described it well. It is around 12" - 16" in length and approx. 2" high and about 1/4" - 3/8" thick. I have not measured it. I noticed my bolts were loose and when I tried to tighten them up they were not getting any tighter and I noticed the hole the bolt goes through was broken.
