I have a 14' v hull jon type boat. Very old. The rails are not very wide and I am wondering how I can install cleats for the side. Any suggestions?

you can install them on the inside at the top of the gunwale, 3/16th ss carriage bolts from the outside, cap nuts on the inside, maybe a support plate
