A friends wife purchased this for whatever reason at a garage sale and is now trying to figure out what it is.  Any help appreciated!

I believe that is a face ripper off'er - I beleive that they are used by the commer slasher / serial murderer!

That is a handle with spikes on the bottom and a fork in the front that was made in Japan.

Looks like it might be an old leather hole punch. Just guessing Are the spikes hollow?
