Buy some cheap mono/fluoro/copoly in 40lb and 80lb test 40lb test mono on smaller hook eye 80lb test mono on larger hook eye

Does your soft plastic extend beyond the hook eye? If so could be the weight pushing back on the worm every time you twitch, jerk, or cast.

Here is another simple remedy, find some flat toothpicks and rig your worm up. Then push the toothpick through the eye of the hook till it stops. Clip off the ends of the toothpick and it will not fall down the hook.
