I'll try to look at mine and see who the company is. There are several places you can google. They either have the forms and sell them or you can send in yours (even in pieces) and they'll make a form for it.

They bought the parts inventory of Champion and carry many of the old windshields. If they don't have the one your looking for they can get you the number of the company that made them for Champion.

Post a picture of your windshields. I ran a 92 charger that I believe had the same or similar design.
I think I have 2 in my shop that I never put on my boat.
