Also, the eyes were really bulging out. Was pretty gross looking, so I avoided touching it as much as I could. Any idea what caused this?

Might have some kind of parasite or skin lice of some sort.

Most likely not disease, a flesh eating disease would usually make more of a hole, that looks like an impact wound. Something like getting bumped against something while spawning (but it's early for that), having a bigger catfish attack it... Most likely it is from getting bashed on the bottom or on cover from when you caught it. I see that on a lot of fish I catch near rocks, it's just usually on the side.

been attacked by another catfish or may have been whacked by a boat prop.
