Shad won't stay alive long at all in this heat. You might get a big gizzard to live a while if you go straight from net to hook.
Perch and baby carp I hook at the back of the dorsal between the dorsal and lateral line, if you hook them too deep and hit that lateral line they die fast. Favorite is probably a big green sunfish with scales scrapped off one side for scent.

I personally target blues and prefer to cut all the fins off of my perch which will kill them pretty quick. Catfish don't like getting stabbed when they're eating. When they swallow whole fish they eat it head first so always hook them on the a$$ end.

Personally, I hook live shad through the nose and sunfish right under the back half of the soft dorsal fin. Shad are kind of a pain in the neck to keep alive with out a proper bait tank, esp this time of year. Although, unless you're Striper fishing this time of year there is no need to really use live Shad w/out a tank but they are great Flathead baits, esp the big ones. If you're using live bait I assume you're targeting Flatheads. They'll be after anything big and lively after they are done spawning. The only time, in my experience, that live Shad are crucial for Catfishing is pre-spawn Flatheads. Which that time has passed for the year. Here in the next couples weeks i'll really be hitting them hard until fall using the biggest, feistiest baits I can find. Bluegills, Green Sunfish, big Shad, small Drum, small Carp, Polywogs, the best in my experience being the first three. Just how it works for me. Don't be afraid to throw a 10 inch bait, I've caught 5lb Flatheads on 6in Perch.

If you are fishing from the bank and using a Carolina rig, where do ya'll put the hook in a live perch?

^^ above the lateral line either way, like G Love said. It allows for the most movement, which is exactly what you want out of any live bait. If the perch, blue gill, etc, expires on the line, no biggie; you turn live bait into cut bait with a couple of quick trims with a sharp blade.
