I keep everything in a small backpack. 4 reels, my shoulder bag for my flies and leaders, a lanyard with all of my "stuff", including tippet, all of my extra fly boxes. In a front pocket are sunscreen and bug repellent. All I have to grab is the backpack and whichever rod(s) I'm taking with me.
Most of the time my lifejacket is wrapped around the backpack, so I don't forget it if I'm bringing my kayak.
When I get to the water, if I'm not paddling, I grab the lanyard and shoulder bag, string up my rod of choice and away I go. If I'm paddling, I put the lanyard over my PFD and throw the shoulder bag behind the seat. The backpack gets stuffed behind that in the tie downs.
I always have my hat on, and my glasses are photochromic, so I have that covered all the time.
